To work with individuals who use our services to build and maintain their safety and welfare. We will provide a package of care for each individual supported that contributes to his or her overall personal and health care needs and preferences. We will work in partnership with other services and professionals to help to maximize each supported individuals independence and to ensure the individuals maximum participation in the community.
Before we provide services, we will ensure that all prospective individuals needs and preferences, are assessed and, working with the individual, we will provide a Care – Support Plan to their approval. We aim to ensure that the care and support Contour Care provides meets these assessed needs of each individual and will be re-assessed as often as necessary.
We will also ensure that we have the flexibility to respond to any changing needs or requirements with the care and support provided.
