- Focus on individuals we support with Person Centred Care: Ensuring that care is tailored to the individual’s needs, preferences, and lifestyle, promoting dignity and independence. We aim to provide Personal Care and Support in ways which have positive outcomes for them and promote their active participation, including the administration of any prescribed medication.
- To ensure that we are fit for purpose, we will examine our systems constantly to ensure that we are successfully achieving our stated aims and purposes. We welcome and encourage feedback from individuals who use our services and their peers, friends and family members.
- To work with individuals who use our services to build and maintain their safety and welfare. We will provide a package of care for each individual supported that contributes to his or her overall personal and health care needs and preferences. We will work in partnership with other services and professionals to help to maximize each supported individuals independence and to ensure the individuals maximum participation in the community.
- Before we provide services, we will ensure that all prospective individuals needs and preferences, are assessed and, working with the individual, we will provide a Care – Support Plan to their approval. We aim to ensure that the care and support Contour Care provides meets these assessed needs of each individual and will be re-assessed as often as necessary.
- We will also ensure that we have the flexibility to respond to any changing needs or requirements with the care and support provided.

LEARNING DISABILITIES
Contour Care support people living with learning disabilities. We understand that the challenges people face can vary, ranging from mild symptoms to more severe and challenging traits.
Our support team understand that the support for learning disabilities can vary considerably. We are flexible in our approach and will adapt our support to allow each individual to live their life as independently as possible.
We offer a range of different support for people with learning disabilities that are designed to meet individual needs. This might be help with maintaining independence or providing encouragement to achieve a particular goal. The level of support we can provide will be completely flexible to meet the individuals needs.
We offer support to adults with learning disabilities, who may live in a supported living unit or within their own home. Our staff will help support them to live safely and comfortably along with encouraging them to be confident to make their own choices and decisions.
We will work closely with other professional bodies, housing associations and private landlords on behalf of the supported person if asked to do so.
AUTISM SPECTRUM DISORDER (ASD)
Autism is a neurodivergent disability that results in a wide variety of different support needs. Each autistic person is different. By viewing an autisics persons needs individually we are able to provide the right support.
Autism affects individuals in different ways and can make everyday life much more challenging for them and the people around them. They can struggle with; paying attention, organisation, social communication, interaction, time management and emotional control. These are some of the common factors that people living with autism and those around them can find difficult.
Contour Care will offer practical support and emotional autism support services to create positive outcomes, what ever the situation. Whether its to offer guidance to help and support, to making the first move out of the family home or seeking steps to further independence.
Our support staff have the knowledge and experience to understand the functions behind behavioural communication, and we aim to help people with autism develop independent life skills, improve communication and resolve environmental issues.
Where individuals may have been previously described as exhibiting; Challenging Behaviours in certain situations, we believe in taking a proactive approach and turning the focus into understanding what the individual is trying to communicate.
We will take lots of time to get to know each person and understand what does and doesnt contribute to postive outcomes. We will also spend time with family members and care professionals close to the individual, to gain understanding of the challenges the person may already have overcome and their hopes and aspirations for the future.
Once we have consultated with the individual, family and healthcare professionals, we will help to create a support plan. Each person will have a unique set of development objectives relating to their autism, which we will focus on when providing support. Our aim is for people to self direct their support as much as possible, helping them to exercise choice in all aspects of their daily lives. We will also encourage each person to be part of their local community in whatever ways suits thems, whether thats getting out and about to do shopping, meeting friends and family etc.
We offer a range of support for people living with autism that is designed to meet an individuals needs. This might be helping with maintaining independence or providing encouragement to achieve a particular goal. The level of support we provide is completely flexible, whether its just a few hours a week or 24-7 support.
DEMENTIA AWARENESS
Dementia can affect people in different ways. Some might lose their memory, others may become unsteady on their feet. There may even be people who have a change in personality.
ALZHEIMERS DISEASE: This occurs through a build-up of two proteins that damage brain cells and affects the transmissions of messages. Over time, this leads to the death of the brain cells, 60-80% of people who develop dementia will have this specific condition.
VASCULAR DEMENTIA: This results from a shortage of blood to the brain caused by conditions such as strokes and high blood pressure. 5-10% of people who develop dementia will have this specific condition.
FRONT-TEMPORAL DEMENTIA: Results from the build-up of proteins in the frontal and temporal lobes. These protiens clump together and damage the brain cells, which eventually die. This area controls emotion and behaviour. 5-10% of people who develop dememtia will have this specific condition.
